In Columbus,Ohio,VITA Treatment Center (Methadone Clinic) had an agency policy that every client had to share the two scales. The scales are agency specific and probably will not make sense to those who are not familiar with heroin rehabilitation.  However, these are examples of two good scales that were useful.
